DIAA SPORTS PHYSICAL & RANK ONE INSTRUCTIONS
ALL online forms must be completed on Rankone.com for the physical to be complete.
Summary of steps
- Using a printed version of the DIAA physical form, obtain a physical for your child on or after April 1, 2026, for the 26-27 school year.
- Create an account at Rankone.com and follow the steps to attach your child to the account. Incoming freshmen will be available starting July 1, 2026.
- Complete ALL the following forms online at Rankone.com
- Transfer - EVERYONE must answer the Transfer question - DO NOT SKIP
- Participation Interest - Indicate which sports your child might participate in during the school year.
- Medical Card - Upload the completed Medical Card (page 5 from DIAA physical form) The physician or nurse practitioner that conducted the physical MUST sign.
- DIAA Acknowledgment Form
- Consent Form
- Emergency Card - Emergency contacts for your child.

FALL SPORTS SEASONS

1. You will need to have your new DIAA sports physical submitted and approved on Rankone.com prior to try-outs/practices. There are links below to register for the Delcastle Wellness center and directions on making an appointment. You can also go to your own physician or Med Express type facility.
All athletes must have a completed DIAA physical form prior to the first practice.
PHYSICALS FOR THE 2026-2027 SCHOOL YEAR MUST have a physical form signed by a licensed physician (MD or DO), a licensed nurse practitioner, or a licensed physician's assistant and dated ON OR AFTER APRIL 1, 2026 stating that the student has been examined within the last 12 months and is medically cleared to participate.
To register for the Delcastle Wellness Center to obtain a sports physical please register using the following link https://redcap.christianacare.org/surveys/?s=XXP8NDAA9F. Once this is received, you will be contacted by email or phone to schedule an appointment. No walk-ins are permitted at this time.
You will need to print the NEW DIAA form using the link below; a parent/guardian must complete and sign this form prior to the appointment and you must bring it with you to the scheduled appointment. https://education.delaware.gov/diaa/health_and_safety/medical_forms/
Parents must create a Rank One account and complete all 6 forms/steps online. Detailed directions are further down on this home page.
2. Sports specific information. Please review the "Team Info" page found on each sports page for specific information such as practice/tryouts dates, times, equipment needed etc. Below are the list of fall sports and the Head Coach's email address for any additional questions.

Get Your Sports Physical @ DTH Wellness Center
All student-athletes must have a valid DIAA Sports Physical completed after April 1 to try out for any sport. Reach out to our partners at the Delcastle Wellness Center to complete your DIAA Sports Physical.

The Delcastle Wellness Center is more than just a place to get your DIAA Sports Physical—it’s a full-service health and wellness hub designed specifically for students. Here are additional Health & Wellness Services for our student-athletes to help you stay healthy, both physically and mentally:
- Medical Care (Nurse Practitioner / Physician): DIAA Sports Physicals and routine check-ups | Minor illness or injury treatment | Answers to your health questions
- Mental Health Support (Social Worker / Counselor): Private, judgment-free counseling | Help with stress, anxiety, grief, or other emotional concerns | Information on mental health & referrals if needed | Small group therapy and peer support opportunities
- Nutrition Guidance (Registered Dietitian): Personalized nutrition plans for athletes | Sports-specific meal planning and fuel strategies | Support with medical diets (diabetic, low-fat, vegetarian, etc.) | Eating disorder resources and education | Meal planning tools and healthy cooking tips
Student-athletes are encouraged to take advantage of the Wellness Center for their required DIAA Sports Physicals before tryouts. Appointments are available throughout the year.
DIAA Physical Info + Voluntary Athletic Insurance
Important Reminder:

Please return the physical to the Main Office PRIOR to the first day of tryouts for your sport. Make sure that you make a copy of all the forms. Please use the form on the link below or you can pick one up in the main office, the nurse's office, or the Wellness Center. Make sure all 12 signatures are in place, if not the forms are invalid. You will not be allowed to participate in any activities until all necessary forms are complete.

Game Fees
The entrance fee for games is $5.00 for all spectators.
We charge for Football and Boys' Basketball. Be prepared to pay at all away events.
Fans must be prepared to pay at away events and any state park fees for entrance, parking, etc.
